what does o2 actually charge if I have the international calling bolt on for USA beginning 00800?

Actually 00800 is an international free phone number, which is free from landlines, but o2 do charge for calling them, if I recall its the cost of an International call.
This is not a US centric number
Best bet is to use a landline or Skype
All US free phone numbers are 1 800 and are chargable from the UK and you need to call +1 1800 xxxxx anf they would come out of any international bundle

You need to dial 00 1 800 for a toll free US number from UK but the call is free.
