I have a contract with virgin media which includes an O2 sim card.
I have misplaced my sim card, which was never activated, and I am unsure how to order a replacement one as I don't even have the phone number for the missing sim

First off it arrives activated, what you need to do is take some photo ID to an o2 store and they can issue you a new one, or alternatively contact o2 Guide: How to find help & contact O2
