Has anyone successfully submitted a claim? All my required documents I submit keep being told not is missing some information. When I look at required documents as per Google promotion, I have never received any of them from O2?

You will need to contact O2 support and ask them to generate a proof of purchase letter. That includes your original purchase date and account details. Since I upgraded online and received no printed paperwork I had to do the same.
They will send you a PDF file which can be attached as documented evidence for your claim. Although it was for Samsung buds pro not Bose headphones in my case.

I managed to submit my claim using screen shots of the email confirmation from o2 with a description of the device on and the order number, as well as the IMEI
