I recently queried some premium rate charges on my account and was assured these would be credited but they have appeared on my latest Invoice and no £5 credit as promised

This is a customer to customer community, not O2.
You need to contact O2.

Look in the recent charges section of MyO2's billing area, @Whythehell - Guide: How do I get to the 'Download Bill' page? (2 below the green highlighted area in the screenshot below, @Whythehell) The credit will be there, I had a refund applied that way earlier in the year, using O2 Twitter team to get it:
