Claim for earbuds

Hi all,

I recently got myself a Samsung FE 2021 and I am aware that I could get my earbuds and something about £100.

Just not entirely sure how to go about claiming it. I think there is a form that I need to fill in. Would someone be able to point me in the right direction?
