If the strange icon is a result of a jailbreak. Then updating the software to the latest version completely removes the jailbreak. Do the update through iTunes on the computer though as the update file when done on the iPhone is much smaller than the much larger update file that is used when its done through iTunes on a computer.
For example, the first iOS 6 jailbreak (and some of the .1 updates), it was possible from what I've read, that the jailbreak was still present when going from certain .1 updates to another. I'm almost certain that iOS 6.1.3 removes the jailbreak. So updating the software to iOS 6.1.3 (which is the very latest software) should completely remove the strange icon.
If my earlier suggestions don't work, then updating to iOS 6.1.3 (through iTunes) and restoring the iPhone as new should definitely remove the strange icon (I would be very surprised if it didn't)
I've jailbroke my iPhone on numerous occasions, but I was using the Sky Go app, after the jailbreak, the app would no longer run and gave the message that streaming is not possible on a jailbroken device, I have friends who have the app too on their iPhone's and Galaxy S2's and they were getting the same message too (my friends S2 had been rooted, which is basically the equivalent to a jailbreak on an iPhone, as far as I know). I know its not the case for everyone, but I find that with going from one major iOS version to another, the need to jailbreak becomes less and less.
The only thing that I want to see on the next iOS update, is a quick access to switch on/off wifi, bluetooth, airplane mode, from within the swipe down notification centre. From what I've read, iOS is going to be in for a big change (appearance wise, but I could imagine feature wise too) as Jonathan Ives is in charge of it now. Jonathan Ives is the guy who designed the iPhone 4 (and some of the others too) he also designed the latest iMac too.

Too be honest I havent seen this icon before.
Try this first:
1: Delete the app off of the iPhone.
2: Do a hard reset on your iPhone. To do this hold the sleep/wake button and the home button, keep holding until you see the white Apple logo appear and then release both buttons.
3: Load up iTunes on your computer and in the apps section, find the You Tube app, right click on it and choose delete. Then restart your computer.
4: Load up iTunes, and redownload the You Tube app again. Once its finished downloading the You Tube app, connect your iPhone and sync and see if the strange icon is still there.
If the above doesn't work then the only thing I could suggest (as you seem to have tried everything I would have suggested) is try restoring from a backup, if the strange icon is still there, then try and restore as new. I would have thought that if all else fails then the restore as new should solve it.
