Sounds like a SIM failure to me.
I'd pop into your local O2 store and get a new SIM provisioned for his account.

Hi.

Have you tried another O2 SIM card in it ?

You could ring O2 on 4445 to ensure the PAYG account is ok.

Ensure the sim tray is clean and free of dust/lint.

You could arrange an appointment at your most local Apple Store Genius Bar. It's out of warranty but I've known a genius to still take a look at no charge.
